To find a single review, group by recording number and template name.
Please note that the records are pipe delimited.

Score | Points **| Answer Index | Comments | Question type *** |

*Answer type
If the question type is positive, the values are as follows
0 = No/Yes
1 = No/Needs Attention/Yes
2 = Poor/Below Average/Average/Above Average/ Excellent

** Points
If the question type is positive, the points are the total number of points
that can be scored by this question.
If the question type is negative, the points are the total points for the
negative category.
If the question type is total fail, the points will display 0.

0 = Positive question
1 = Negative question
2 = Total fail question
